S\u00f3lvangur is a centre for services and preventative support for older people.<\/strong><\/h4>\n

The first clients at S\u00f3lt\u00fan Health Centre have arrived, although the centre only formally opened on 1 September. S\u00f3lt\u00fan Health Centre <\/a> offers a new type of specialised service designed to provide rehabilitation for older people through the interdisciplinary teamwork of healthcare professionals. The aim is for people to be able to remain in independent living for longer with a better quality of life, and the service is designed to enhance the individual's well-being, functionality and abilities. The new service is provided on the 3rd and 4th floors of the beautiful, refurbished premises of the former S\u00f3lvangur in Hafnarfj\u00f6r\u00f0ur. It will be able to accommodate up to 39 individuals at a time.<\/p>\n

Individualised, holistic and multidisciplinary rehabilitation<\/strong><\/h4>\n

S\u00f3lt\u00fan Health Centre accepts individuals aged 67 and over who have received a referral from Home Care Nursing, Health Service of the Capital Region, and Home Care Services of Reykjav\u00edk, and who have been professionally assessed as being likely to achieve a significant improvement in their ability for self-care. and that the individual will benefit from the treatment. The service is fully subsidised by the National Health Service, but each person is expected to stay at the centre for 4 weeks and receive targeted, interdisciplinary training and education from a diverse group of healthcare professionals and other specialists. The rehabilitation programme includes education, consultations with specialists, individual and group training, health-promoting training, activities and social events. This new service is intended to reduce the likelihood of ill health requiring A&E services, home care, or leading to premature hospitalisation. ,Our aim is to graduate stronger and more independent individuals who can enjoy their later years with improved health.\u201c <\/em>says Halla Thoroddsen, managing director of S\u00f3lt\u00fan Elderly Care Services.<\/p>\n

S\u00f3lt\u00fan Health Centre is a joint project of the Ministry of Health, Sj\u00fakratrygginga \u00cdslands, Hafnarfjar\u00f0arb\u00e6jar and S\u00f3lt\u00fans \u00f6ldrunar\u00fej\u00f3nustu, which has run the S\u00f3lvang nursing home in Hafnarfj\u00f6r\u00f0ur and the home care service S\u00f3lt\u00fan Heima, where the emphasis is on health promotion in the home.<\/p>\n
