Holistic Service
Organised and holistic service which aims to ensure quality and promote continuity of care for an individual, and is provided by the service providers best suited to meet the individual's needs at any given time.
Integrated service
Organised and continuous service which aims to promote a child's well-being and is provided by them

Holistic services for individuals with multiple support needs
When an individual's request for a holistic service to begin is received, the adviser/case manager can obtain information about the individual's circumstances from a multidisciplinary service team, service providers, and those providing general services for the individual. The aim of information gathering at this stage is for the adviser/case manager to gain an overview of the individual's case and thereby have the basis for providing a holistic service.
Holistic service involves a counsellor/case manager working with information about an individual's circumstances and, where appropriate, liaising further with service providers and others.
If an individual requests a comprehensive service and there are grounds to believe that they require multi-faceted support from multiple service systems, the local authority where the individual is domiciled shall appoint a case manager in order to ensure quality and continuity of service to the individual. Service providers and the individual themselves form a multidisciplinary service team led by the social services. Others involved in supporting and assisting the individual can also be part of the multidisciplinary service team.
Explanations
Information about an individual's circumstances
Processing of information about an individual's circumstances is permitted for advisers/case managers and those providing general services, in order to ensure quality and continuity of service to the individual. Processing of personal data includes, amongst other things, the collection, recording, retention, disclosure and matching of information about an individual's circumstances, including sensitive personal data and information about criminal conduct and suspected criminal conduct. Proportionality must be observed in the processing of personal data under this Act, and personal data shall not be processed beyond what is necessary in each individual case for the purposes concerned.
Multidisciplinary service team
An interdisciplinary service team is one that provides holistic/integrated services on behalf of the state or a local authority, and/or a private party that provides similar services under a service agreement. Examples of service teams include nurseries, primary schools, after-school clubs, youth centres, secondary schools, general practice, specialised healthcare services, the police, and social services. The team's role is to create a support plan, arrange services, consult on the service, and ensure continuity and quality.
General service
All organised work carried out by parties other than the service team. Independent specialists,
Sports, arts and youth activities, voluntary associations and interest groups.
Consultant/Case Manager
The adviser/case manager's role is to provide the user with guidance and advice, as well as to ensure they can make use of the services and support of community organisations and other support from their social network. When consent has been given to create a support plan, a case manager is appointed in consultation with the user.
The adviser/case manager is responsible for drawing up a support plan and for managing the support team, where applicable. They ensure that services are provided in accordance with the support plan/individual plan. He provides advice and information to the support team members on holistic services for the individual.

Personal data protection and processing
By signing, an individual declares that the processing of information about their circumstances is permitted for the adviser/case manager. They are therefore authorised to obtain information from those providing general services and other service providers to ensure quality and continuity of service. The purpose of processing the information, which includes its sharing, is to provide individuals with a comprehensive service.
Processing of personal data includes, amongst other things, the collection, recording, storage, disclosure and comparison of information about an individual's circumstances. This includes sensitive personal data and information on criminal convictions and alleged criminal offences. Depending on the type of service, the processing of information may relate to the following categories of data concerning the applicant.
General personal data: Personal details (e.g. name, national insurance number, home address, telephone number, email address, family status), financial and property information, social status and circumstances, and other information provided by the applicant during the service application process.
Sensitive personal data: health information.
Due regard shall be had to proportionality when processing personal data under this Act, and personal data shall not be processed beyond what is necessary in each individual case for the purposes concerned.
The processing of the aforementioned personal data is based on the individual's consent, in accordance with item 1 of the first paragraph of Article 9 and item 1 of Article 11 of the Personal Data Processing Act No. 90/2018.
Hafnarfjörður Municipality safeguards the security of personal data with appropriate organisational and technical measures, including access controls and encryption. All employees who provide services on behalf of an individual are bound by confidentiality in accordance with Chapter X of the Administration of Law, concerning information they have become aware of in connection with tasks under this chapter and Chapter V.
Individuals who have requested a holistic service can withdraw their request at any time. Withdrawal means that the service the individual receives will no longer be holistic. Notification of withdrawal of consent should be sent to the Family and Children's Services Department of the City of Hafnarfjörður.

Children and their families
On the integration of services pursuant to Chapter V of the Act on the Integration of Services No. 86/2021
All children and parents shall have access to the service of a liaison for the integration of first-level services.
When a request from the parent(s) and/or the child for the integration to begin is received, the contact person may gather information about the child's circumstances from service providers and those providing general services for the child's well-being. The aim of the information gathering at this stage is for the coordinator to gain an overview of the child's situation and thereby have the basis for leading the integration of services.
Coordination involves a contact working with information about a child's circumstances and, as appropriate, having further contact with service providers and others. Once coordination has been established, service providers and those providing general services in the child's best interests can share information with each other for the purpose of coordination, i.e. without the information having to go through the contact person.
If a request is made by the parents and/or the child for the integration of services, and there are grounds to believe that the child will require multi-faceted secondary and/or tertiary level services for the long term, the local authority where the child is ordinarily resident shall appoint a service coordinator in the child's best interests. The case manager shall establish a support team, which shall include representatives from service providers who provide services to the child. Those providing general services for the child's well-being may be invited to join the support team if required.

Service provider
A provider of welfare services on behalf of the state or a local authority, and/or a private party providing similar services under a service agreement. Service providers include, for example, nurseries, primary schools, after-school clubs, youth centres, secondary schools, general practice, specialised healthcare services, the police, social services and child protection.
General service
All organised activity run by parties other than service providers. Self-employed professionals, sports, arts and youth activities, voluntary associations and interest groups.
Contact person
From birth until a child starts nursery or primary school, the contact person is a member of staff from the health centre or health facility in the child's health district. The same applies if the integration of services is required during pregnancy. When a child is in nursery, primary or secondary school, the contact person is a member of staff at the school the child attends. The contact person's role is to provide information about services to support the child's well-being, ensure access to primary assessment, and support the integration of services from all service providers. Director of Studies
The person who leads the integration of services in the best interests of a child who has a need for secondary or tertiary level services. The case manager has the child's best interests at heart. The role of the case manager is to provide advice and information about services in the child's best interests. Assist in ensuring access to an assessment and/or diagnosis of the child's needs. Proportionality must be observed in the processing of personal data under this Act, and personal data must not be processed beyond what is necessary in each individual case for the purposes of the processing.
The processing of the aforementioned personal data is based on a legal basis, in accordance with point 3 of the first paragraph of Article 9 of Act No. 90/2018 on Personal Data Protection and Processing of Personal Data, and the legal basis for the processing is found in Article 23 of Act No. 86/2021.
Service providers and those providing general services in the interests of a child's well-being are bound by confidentiality with regard to information they have become aware of in connection with tasks under this chapter and Chapter V.
Individuals who have requested integrated services can withdraw their request at any time. Withdrawal means that the services the child receives will no longer be integrated. A request to withdraw consent should be submitted to the relevant service provider or to those who provide general services for the well-being of children. When a child's circumstances change, e.g. they move between school stages or to another local authority, the previous contact person must ensure that the new contact person receives the necessary information about the child's circumstances.
